False Super Saiyan
This form is the result of a Saiyan being unable to increase their ki level enough for the body to complete the transformation to Super Saiyan. Even though the form is incomplete, their are certain Super Saiyan features that appear, such as the elevated hair and yellow ki aura. Another feature includes no pupils. Because their where unable to complete the transformation they used up much more energy. Because of this they require to revert back to their regular state for a short time. Goku only used this form in the 4th movie and in the anime filler saga when he fought the Ginyu Force in Other World.
This is not the name of the transformation, in fact it was never named in the film it appeared; Lord Slug. In was referred to as "Super Saiyan" by King Kai.

Super Saiyan
Details: Goku reached this first stage in the Frieza Saga, after Frieza nearly killed Piccolo and then killed his best friend, Krillin. That was the event that created the rage within Goku completing his requirements to become a Super Saiyan (see saiyan's section for details). the first time a Saiyan reaches this level it happens generally on it's own when the saiyan's reaches uncontrollable emotion; in most instances, rage. However after words a Saiyan can usually transform on his own.
Several physical chances occur when a user becomes Super Saiyan. The most noticeable change is the hair, which states up on end and becomes a golden yellow. The color of the Saiyan's eyes becomes a shade of green and their muscle mass increases. A Super Saiyan's aura is also very unique, instead of being blazing it quite simple and appears in a very gentle wave.
The saiyan's behavioral changes greatly, as the saiyan comes to possess heightened aggression from their normal state of mind. The first time this happen was when Goku transform. From training, these negative effects can be suppressed and controlled, resulting in a super saiyan who can suppress his energy and act as if he was not transformed.
Other notes about this form is that because Cell show signs of the same kind of aura many have considered him to being able to be a super saiyan being that he had saiyan cells.

Super Saiyan 2nd Grade
Known as Ascended Saiyan is a transformation that occurs when a Super Saiyan has further the transformation of his body. The result is a Super Saiyan that is much stronger and faster then a regular Super Saiyan. Unlike the normal transformation, this one happen simply by increasing their ki.
In this advance state, the Super Saiyan appears with longer, finer hair that is forced further upward. The muscle mass is also increased significantly from their regular Super Saiyan form. The aura chances greatly from the gentle wave to a more lazing uncontrolled wave. The aggression of the Saiyan is also furthered increased.
Super Saiyan 2nd Grade isn't a true ascended Super Saiyan but an enhanced version of the normal Super Saiyan form due to a few basic limits and characteristics. This form was only seen in the early fight with Cell, used by both Trunks and Vegeta. Goku also briefly reached this level while he and Gohan trained in the Room of Spirit and Time.

Super Saiyan 3rd Grade
In this advance form of Super Saiyan 2nd Grade, officially named in the manga by Trunks as Super Super Saiyan. It's mostly commonly known as Ultra Super Saiyan by many fans. This transformation is very similar to that of the 2nd Grade form. The main difference is the fact that the form does not happen naturally. In order to reach the maximize strength of this form the user must increase his ki to full capacity. It is this that creates the forms greatest flaw and makes it for the most part unless.
The main reason this form is considered to be insufficient for battle is due to the muscle mass itself, which has move to a point that it actually inhibits the Saiyan's speed. It was Goku who first stated the weakness of the form to Gohan; however it was later learned that Vegeta also reached this form and discovered its weakness and there for stop his training of it. This was revealed by Trunks after Cell exposed the weakness to him.
Super Super Saiyan form appears as a Super Saiyan with extreme muscle mass and wild hair, with the locks of hair becoming more and more firmly upright than the previous forms. There is now a blazing aura that surrounds the Saiyan.

Full Power Super Saiyan
This is a form of Super Saiyan that is very unique, in which that it's not an alternate form of Super Saiyan but a state of power achieved thought the mastering of the normal state of Super Saiyan. Once a Saiyan fully masters the Super Saiyan form this state becomes the regular Super Saiyan transformation.
This form is achieved by training the body to overcome the negative effects of the Super Saiyan form. They are both ki consumption and behavioral alterations from the Saiyan's untransformed state. This allowed the Saiyan to be able to fight longer with out wasting energy transforming into Super Saiyan. Goku first thought of mastery of the Super Saiyan level when he and Gohan where training in the Room of Spirit and Time.
The state in which they reach was that of to feel natural as a Super Saiyan. At the end of their training, exiting the Room of Spirit and Time, Gohan was the first to walk out, in this Piccolo first mistaken him for Goku, not believing Gohan could really reach such a level of power. While only Goku and Gohan are confirmed to have achieve this, in the manga it was suggested that most of the other Super Saiyan's also achieve it by the time of Majin Buu Saga.

Legendary Super Saiyan
This form appears only in the Dragonball Z films, as a result was not in the manga. This is a very unique form of Super Saiyan due to the ability to it being genetically selective and exclusive to the character known as Broly.
Beyond that regular Super Saiyan form, Broly's final form greatly increases his body size and muscle mass. Even more so then a 3rd Grade form and with out the loss in speed. Broly's pupils seemingly disappeared. What also makes this form so unique is that it doesn't share any of the physical limits and flaws of a normal Super Saiyan, more like a counter of them. As time passed he got stronger, which in the case of a normal Super Saiyan would lose power the longer he fought.
Many times by fans is is misinterpreted that Broly's hair is naturally a tint of green. This was merely the effects of the device that was used to control him early in the film. As seen in the later films the green tint is not present. It is also believed that this form is in fact a 4th grade of Super Saiyan which passes the limits of a 3rd grade. There are many facts to support the claim, however it was never officially stated and is mostly known as Legendary Super Saiyan.

Super Saiyan 2
A fully ascension of the normal Super Saiyan Level, like the normal form, this form occurs on it's own and can't be done by simply wanting it to happen. While this form does require more control of the ki to happen, then that of normal Super Saiyan. The first person to achieve this level was the half-saiyan Gohan in his fight against Cell.
Super Saiyan 2 possesses the same basic characteristic of Super Saiyan. Only a slight difference are noted from normal form, which is a slight growth in muscle mass. Other physical changes include longer, spikier, upright-standing hair. The aggression effects are the same as a unmastered Super Saiyan. The power, strength and speed is increase substantially from Super Saiyan. As a result in a more wider aura that blazes around which mostly is accompanied with bluish-white electrical bolts.
Gohan's behavior in Super Saiyan 2 forum is often noted to be quite different than Goku's or Vegeta's in the same form. After becoming Super Saiyan 2, his demeanor drastically changed from gentle to sadistic. Such as Gohan wanting cell to suffer for his crimes where as before he refused to harm Cell. As noted this was not seen in either Goku or Vegeta when they reached Super Saiyan 2.
In the anime series, there are at times great difficulty distinguishing between the first and second Super Saiyan levels due to inconsistencies that Toei created by placing bolts in the normal Super Saiyan forms, such as Gotenks first appearing, Vegito and even at times Goku and Vegeta. In the manga it was much easier to tell the two apart base on the form possesses bolts within the aura.

Super Saiyan 3
This is the strongest and most powerfulest of forums in the manga and Dragonball Z anime. This transformation is unlike normal Super Saiyan forms because like Super Saiyan 3rd Grade it is force on rather then it happening naturally. Because of the extreme amount of ki used to keep the transformation the Saiyan always over-exert's themselves once fully transformed. Generally leads to the Saiyan becoming exhaustion and reverting back to normal Saiyan state.
Super Saiyan 3 has waist-length variation of Super Saiyan hair, their eyebrows also disappear. The Saiyan's eyes maintain the same color, however Goku's eyes were altered in the anime to have Black pupils with green irises. The bolts of electricity sparking are a carry over from Super Saiyan 2. While this form has a major disadvantage, it still increases speed and power more then Super Saiyan 2.
While Goku was in Other World, he could maintain this form for as long as he wanted due to the unlimited supply of energy he possessed. Once he return to Earth his energy capacity was limited, which resulted in his not being able to maintain the transformation for long periods of time. Even as Goku was fully grown when he used Super Saiyan 3, Gotenks (the only other character to be seen using this form) appeared to have the ability to maintain the form as long as Goku could. This could be explained by the effects of almost unlimited power by the Fusion Dance.
In Dragonball GT, Goku also used this transformation while as a child, but his child body didn't have the fundamental requirements to maintain the transformation. To the result, his body reverted back after a few minutes of being Super Saiyan 3. Later after regrowing his tail he seem to be able to use the form longer, while it still had no effect and he tired out and revert back to normal, this could simple due to a mistake by Toei.

Super Saiyan 4
This is an anime only transformation that appears only in Dragonball GT. As a result it never appeared in the manga. Super Saiyan 4 is thought off to be the ultimate form of the Saiyan race and is implied to only be accessible by pure blooded saiyan's, this was discovered when Baby who was in Vegeta's body try to transform to this form but was not able to, being that Baby made Vegeta's heritage impure
In this form, the Saiyan's hair is not golden yellow as with the first 3 levels of Super Saiyan. However while the color vary, often it was simply their natural color. The Saiyan's red tail is present and their bodies are covered in a shade of red fur. Their top and bottom eyelids become red and the hair is lengthened half way, almost 3/4 down the back and becomes untamed. The Saiyan's maintain their pupils but like the hair color, the eye color can also vary. The best way to sum this form up would be that of a humanoid Saiyan with Oozaru characteristics.
Super Saiyan 4 is only achieved when the primal extremely strong forceful urges of the Golden Oozaru are suppressed. After this form is first achieved, the saiyan can transform with out becoming a Golden Oozaru first. This form was first reached by Goku while fighting Baby Vegeta. Later Vegeta achieve this also but it was only temporary due to it being achieved unnaturally. Vegeta was artificially exposed to high concentrations of Bruitz waves by a machine Bulma created.
You will find many fans have theories that this is in fact not the true Super Saiyan 4 form and more of a 3rd level of the Oozaru form, with normal color Oozaru being the first, and Golden Oozaru as the 2nd. While there is no prove to disclaim this there is little to support it. In any event Super Saiyan 4 is in fact just that, even know it does not follow Super Saiyan 3.
